Cities Most at Risk for Severe Illness Due to Coronavirus

The coronavirus continues to spread across the United States, affecting communities large and small. The spread has been uneven geographically, but some populations of people are more vulnerable to experience serious illness if they were to catch the coronavirus compared with others. According to guidance from the CDC, some of the factors that make serious illness more likely if an individual catches the coronavirus include chronic health conditions as well as advanced age.

In this study, we examine the 500 largest cities in the United States, using data from the CDC’s dataset 500 Cities: Local Data for Better Health, as well as data from the U.S. Census Bureau’s American Community Survey. We looked at six health conditions, as well as the population’s age in each city, in order to analyze cities that would be most affected by serious illness should a coronavirus outbreak occur in the city. The six chronic medical conditions studied are obesity, diabetes, kidney disease, chronic obstructive pulmonary disease (COPD), high blood pressure, and heart disease. Additionally, we examined the percentage of the population aged 65 or older. Having these conditions or age does not make an individual more likely to catch the coronavirus, but does increase the likelihood of severe illness if an individual catches the disease.

Our study created a vulnerability score for each city that takes into account the percentages of people in the population who have each medical condition as well as those aged 65 or older. We ranked the top 100 cities based on this vulnerability score.

We found that 54 of the 100 most vulnerable cities were located in the Southern United States. Florida was the state with the highest number of cities (17 of the top 100), owing to its high number of seniors in the state and its large population. Texas was the second-most represented, with ten cities in the top 100. The most common chronic health conditions were obesity and high blood pressure, with diabetes a distant third. Heart disease, COPD, and kidney disease had lower prevalence compared with the other conditions.

The four most vulnerable cities in our study were midwestern cities where over 40% of the population was obese, and over 40% of the population had high blood pressure. These cities were Gary, Indiana (#1); Youngstown, Ohio(#2); Flint, Michigan (#3); and Detroit, Michigan (#4). Methodology

This study examined population medical condition risk factors for serious illness caused by coronavirus in cities around the United States. To understand factors that place individuals at risk, we used guidance from the CDC, which highlighted risk factors based on age and health status. In this study, we used seven of the risk factors highlighted in the CDC guidance, which included six chronic health conditions and one age-related factor.

Six of the chronic health conditions that make individuals more susceptible to serious illness from coronavirus that we used in our study are heart disease, high blood pressure, chronic obstructive pulmonary disease, diabetes, kidney disease, and obesity. To determine the prevalence of these medical conditions, we used data from the CDC dataset 500 Cities: Local Data for Better Health from 2019, which was the latest release available. This dataset included estimates for the largest 500 U.S. cities using data from the 2017 Behavioral Risk Factor Surveillance System (BRFSS) conducted by the CDC. For each city, the dataset estimated the prevalence of each medical condition as the percentage of adults aged 18 or older who have that condition.

Additionally, we used data from the U.S. Census Bureau’s most recent American Community Survey, which used survey data from 2018. We used this data to compute the proportion of the population in each of the largest 500 cities that is age 65 or older.

To measure the vulnerability of each city’s population to serious illness from the coronavirus, we created a vulnerability score that was a composite of the seven conditions, and we then ranked the 500 cities based upon their susceptibility to serious illness from coronavirus based on their population’s age and chronic health conditions.

One important limitation of this study is that it does not account for individuals who have more than one chronic health condition and/or are also age 65 or older.
